I, the undersigned, understand that participation in programs, workshops, and events at Make.Believe. Makerspaces involves engaging in hands-on activities which may include, but are not limited to:

Use of tools and equipment (e.g., 3D printers, CNC routers, laser engravers, electronics, scissors, hot glue, craft tools)
Physical activity (e.g., building, climbing, crafting, light movement games)
Exposure to small parts, adhesives, paints, and other materials

I acknowledge that these activities carry inherent risks of injury or harm, including but not limited to: cuts, burns, eye injuries, allergic reactions, repetitive stress injuries, or accidents due to misuse of equipment.

I voluntarily assume full responsibility for any risk of loss, damage, or personal injury that may occur through participation in Make.Believe. Makerspaces activities, whether supervised or unsupervised, and regardless of fault.

RELEASE OF LIABILITY

In consideration of being permitted to participate in any Make.Believe. Makerspace activities, I hereby release and hold harmless Make.Believe. Makerspace, its owners, staff, volunteers, contractors, and affiliates from any and all liability, claims, demands, damages, costs, or causes of action arising out of or related to any loss, damage, or injury (including death) that may be sustained while participating in any activity on or off-site.

MEDICAL EMERGENCIES

In the event of a medical emergency, I authorize Make.Believe. Makerspace staff to seek emergency medical treatment for my child or the participant named above. I understand that I am responsible for any medical expenses incurred.

BEHAVIOR AND SUPERVISION

I understand that Make.Believe. Makerspace is a shared learning environment and not a licensed childcare facility. I confirm that my child is capable of participating respectfully and responsibly. I acknowledge that disruptive behavior may result in removal from the program without refund.

I have read and understood the above. I acknowledge that by signing this document, I am waiving legal rights I may have against Make.Believe. Makerspace. I certify that I am the parent or legal guardian of the participant(s) or am 18 years of age or older.

I understand that, to ensure the safety and security of all children and staff, Make.Believe. Makerspace is monitored by video surveillance in public indoor areas. These cameras do not record audio and are not installed in private areas. Footage is stored on-site, NOT on a cloud platform. Footage may be reviewed in the event of safety concerns

I grant Make.Believe. Makerspace permission to take and use photographs or videos of me or my child during programs for promotional purposes (including website, social media, and printed materials).
